New Patient Information

To make your first visit more convenient and take up less of your time, new patient forms may be downloaded from this website and filled out at home. Simply bring the completed forms along on your first visit. We will review this information with you when you arrive. To download the new patient forms, click here.

We will then do a history and examination to focus on your particular problem and to see if we may be able to help you. We may need X-Rays to get a better look at your spine, and to help us understand exactly what and where your problem is. This whole process can take anywhere from fifteen to thirty minutes.

When X-Rays are taken we will generally go over them with you on the same day. At this time we will explain what you are looking at and where your particular problem is located. We will then recommend a treatment plan that will fit your individual needs.

Financial Information

Each patient has different financial needs and we will work with each patient to figure out a payment plan, which will be fair for both of us.

We provide care to newborns, children, teenagers, young adults, and mature adults (Yes! Medicare covers chiropractic adjustments).

Types of Treatment

We treat people for pain problems such as headaches, neck pain, shoulder pain, elbow pain, wrist pain, mid back pain, low back pain, hip pain, knee pain, and foot pain or just because they wake up with pain.

Along with pain problems we treat injury problems such as sports injuries, work related injuries, and auto accident injuries.

Hours and Location

You can be seen the same day you call in most situations. We have a twenty-four hour answering machine, seven days a week. We can make house calls if a person is home bound. We are located in the beautiful "Uptown Moline" on 15th Street Place, right next to the giant Moline water tower.

We encourage patients to call the office at 309.797.5700 to set appointments, however walk-ins are always welcome.
